Graduation Procedures

Graduate Handbook

In order to graduate, a series of forms must be submitted as well as the thesis (for MA thesis option) or dissertation. The first deadline is within the first weeks of the semester. The College is very strict about these deadlines and will NOT make any exceptions.

Note: All forms must be typed with original signatures. All forms can be found on the Eberly website at:  Eberly Graduation Forms and Processes

Faculty:

Faculty should contact students who intend to graduate to encourage them to contact the department office regarding the process and deadline dates. It is the responsibility of the main faculty advisor to obtain signatures on the Eberly Thesis and Dissertation Defense Form. See below for details.

Department:

It is the department office’s responsibility to contact faculty and students regarding the graduation process, various necessary forms, and the deadlines for that current term. This includes emails to students and faculty.

Student:

It is the responsibility of the student to apply for graduation and commencement. Students must apply separately for each degree and certificate they are receiving. For specific instructions, see the guidelines outlined on the college website:  Eberly Graduation Forms and Processes 

Students must be registered for at least 1 hour for the term he/she/they plan to graduate.

Please provide the department office with a current non-WVU email address and phone number in case questions come up about your graduation.

Application for Graduation: The application for graduation form can be found by going to your MIX account and clicking on the STAR tab. All applications are submitted electronically and must be done by the due date. 

Plan of Study: (Plan developed by student, advisor and director of graduate studies). This should be completed within the first year of course-work. The student and advisor should meet and complete this form together. PhD students should wait and do this when they have their first-year review. If changes need to be made to the form, this can be done on the original form. If major changes are needed, a new Plan of Study can be completed. This form goes into the student file. Note: Student must collect all signatures

Thesis and Dissertation Defense Date Declaration Form: The department representative must submit this form no less than two weeks prior to the scheduled defense. You can retrieve the form by going to: https://eberly.wvu.edu/students/graduate/graduation-forms-and-process

Complete this form and submit it to the department’s administrative assistant. You do not need to collect signatures.

Eberly Thesis and Dissertation Defense Form: It can be found by going to:  Eberly Graduation Forms and Processes

Complete the top of the form and type the committee member’s names on the left. Take this form to the defense. The faculty advisor collects signatures and submits the form to the director of graduate studies, who sends it to the college within 48 hours of the defense.

Faculty Please Note: Submission of the Eberly Thesis and Dissertation Defense Form does not mean the student has fulfilled the requirements for graduation. The student must still submit the final version of the thesis/dissertation and examination-option MA students must pass their final classes. Please also note that you must submit the Eberly Thesis and Dissertation Defense Form immediately after the defense to the department. The department staff must submit the form within two working days from the defense. 

ETD Submission Form: This form requires signatures of the committee members. It should be taken to the defense and have the committee members sign it then. It can be found by going to:  Eberly Graduation Forms and Processes
